2 lakh new cases of breast cancer estimated per year: Dr Srinivas K G

Mysuru: Breast cancer has become one of the most common cancers among Indian women, with over 200,000 new cases estimated each year and it accounts for approximately 30% of all cancers in Indian women, with an increasing incidence in both urban and rural areas said Dr Srinivas K G, Consultant Medical Oncologist, Bharath Hospital & Institute of Oncology Mysore (Bharath Cancer Hospital, BHIO) .

Addressing media persons at Press Club here on Wednesday Dr Srinivas said “Breast cancer is a significant health concern in India, with rising cases and a concerning mortality rate. Here’s an overview of the current situation, contributing factors, and ongoing efforts in the country;

Risk Factors: Family history and genetic predispositions, such as BRCA1 and BRCA2 mutations, can increase risk; Sedentary lifestyle, obesity, late childbearing, reduced breastfeeding, and dietary habits contribute to higher risk; Exposure to endocrine-disrupting chemicals and other carcinogens may also play a rorole.

Awareness and Early Detection Challenges: Awareness Many women lack knowledge about symptoms, self-examinations, and the importance of early detection; Cultural stigma surrounding cancer, especially breast cancer, can lead to delayed diagnosis and treatment; Access to screening facilities like mammograms is limited, especially in rural areas.

Diagnosis and Treatment: Diagnosis often happens at later stages, with many cases only identified in Stage III or IV. Treatment options in India include surgery, chemotherapy, radiation, and hormone therapy. However, access to these treatments can vary by region, with rural areas facing more limitations. Financial strain is a significant factor; While public healthcare initiatives exist, out-of-pocket expenses are often high.

Government and Non-Profit Initiatives: National Cancer Control Program (NCCP), Launched to increase awareness, screening, and early diagnosis; Ayushman Bharat- Provides health coverage, including cancer treatment, to low-income populations. Non-profits, like the Indian Cancer Society and others, play an active role in raising awareness and providing support to patients. At Bharath Cancer Hospital all state and central government health schemes are accepted for treatment.

Research and Future Outlook: Ongoing research focuses on better understanding genetic factors, improving early detection, and developing cost-effective treatment options. Increasing advocacy and educational programs aim to reduce the stigma and encourage early screening, especially in underserved areas. Overall, while progress is being made, increased public health investment, widespread awareness programs, and accessible health care are essential to combating breast cancer in India effectively.

Briefing about recent advancements focus on improving and preserving quality of life Dr Vijay Kumar M, Consultant Surgical Oncologist and Robotic Surgeon, BHIO said that Oncoplastic surgery ensures complete excision of tumors while considering the surrounding tissue for aesthetic results.

Techniques like implants and tissue flaps help in restoring breast shape after excision. Procedures are designed to maintain or restore symmetry between the breasts, often involving modifications to the opposite breast if necessary. Advances such as nipple-sparing mastectomies and partial breast reconstruction allow for less invasive options and better preservation of natural appearance. Oncoplastic techniques involve a multidisciplinary team, ensuring that surgical decisions align with patient preferences and overall treatment goals.

He also mentioned that robotic surgery in breast cancer plays a significant role in enhancing surgical precision and patient outcomes.

“Smaller incisions, leading to less tissue damage, reduced scarring, and quicker recovery times compared to traditional open surgery. Surgeons can perform intricate maneuvers with greater dexterity, which is particularly beneficial in complex cases, such as those involving multiple tumors or challenging anatomies. Studies suggest that robotic-assisted procedures may lead to lower rates of complications, such as infections and postoperative pain,” added Dr Vijay.

The sentinel lymph node plays a crucial role in breast cancer surgery by serving as the first node to which cancer cells are likely to spread from the primary tumor. The sentinel lymph node helps determine the extent of cancer spread, providing important staging information that influences treatment decisions. The results from sentinel lymph node analysis can guide adjuvant therapies, such as radiation or chemotherapy, by indicating whether there is a need for further intervention.

As part of a month-long breast cancer awareness program, HCG Bharath Hospital and Institute of Oncology conducted various awareness programs including camps at prominent parks across the city, free screening test and 50 percent discount on mammography test. Apart from this BHIO had also organized a talk on breast cancer at various centers and organizations.
